Who can make referrals?

• Physicians, social workers, hospital or nursing home discharge planners, registered nurses, and family members. It is a must to obtain a physician’s order to provide service.

Who is qualified for our service?

• Client must be homebound.
• Needed care must be skilled in nature and can be delivered only by a skilled medical professional.
• Needed care must be intermittent with more than one visit where wellness can be obtained at some level with treatment.

What are our forms of payment?

• Payment may be done through the client’s healthcare coverage with Medicare, Medicaid, private insurance and/or self pay.
