Services

Wellness Home Care, Inc. provides direct skilled nursing care and home health aide services to homebound elderly patients. It also provides therapy services from trained and highly qualified healthcare professionals in physical therapy, occupational therapy, speech pathology, and medical social work.
Skilled nursing care is provided by our registered nurses. They go out and visit patients in their homes. We also have a designated registered nurse who is on-call 24 hours a day, 7 days a week in case a patient would need some assistance with his or her care anytime.
Skilled Nursing Services include:
1. Initial and Periodic Nursing Assessment of the Patient and Caregiver
2. Patient/ Caregiver Education
3. Pain and Coping Management
4. Medication Administration and Management
5. Nutrition Education and Management
6. Education and Monitoring provided for Decubitus Care, Ostomy Care, Enteral Feeding and Care, Oxygen Therapy Management, Catheter Care and Maintenance, Nebulizer Treatment, Tracheostomy Care and Hydration Management
7. Blood Draw and Laboratory Monitoring
Rehabilitation Services are provided by the following healthcare professionals and certified para-professionals: physical therapists, occupational therapists, speech-language therapists, medical social workers and home health aides. They are available to help you with your therapy and personal care needs:
1. Physical Therapists
They provide therapy that includes gait training, cane, walker and wheelchair instruction. They also provide therapeutic exercises for relief of joint and muscle pain and restore muscle function.
2. Occupational Therapists
They help patients improve their capacity for independent living. They assist you with how to go about self-care such as bathing, dressing, eating, personal hygiene and housekeeping.
3. Speech-Language Therapists
They help clients with speaking, hearing and swallowing problems.
4. Medical Social Workers
They provide patients and family members with understanding and coping strategies for problems caused by illness or injury. They also help patients utilize community resources such as legal, financial, meals, transportation and homemaker services.
5. Home Health Aides
They are trained and certified nursing assistants who can assist patients with activities of daily living such as meal preparation, hygiene and grooming.
